Convert a MediTracker Record to a Care Plan Record
Accessing MediTracker patients in Inca Integrated Care Platform
Sign in at inca.health (if you do not know your sign in details please contact Precedence Support
(T: 1300 236 638)
- Your patient list will be displayed when you sign in
- Click MediTracker to view patients using MediTracker
- To access an individual patient click on the patient’s name
The GP symbol next to a MediTracker patient’s name indicates they already have a care plan record
To navigate back to the list, click Patients
Converting a MediTracker record to a care plan record
- Open the patient record in your practice software
- Click the Precedence Connector icon in the taskbar and select Check Status
- Click Create in Inca in the popup
- Confirm patient consent to sharing their health record
- Click Create Health Record
It will appear that you are creating a new record in Inca; the new record will merge with the existing record automatically
