Creating a Medicare Care Plan (GPMP 721 / TCA 723)

info.png NOTE: The patient must already be created in Inca to proceed with creating the care plan. If you have not already created the patient, please refer to the guide to Create a Patient Health Record. 


Start Care Plan Creation workflow

1. From the Patient Overview tab select Create Care Plan
2. Select care plan type being created (GPMP + TCA)
3. Document patient consent and select
Next
 

Select Conditions

4. Select the conditions applicable to the patient and select Next

If a condition is not listed, add a Custom Condition by typing the condition in the ‘Custom Condition’ text box and selecting Add

 

Select Care Team

5. You are able to add, edit or remove members of the care team. To search for a new provider for the care team, select Add Care Team Member
 
6. When you have chosen all the correct providers, select Next

Edit Care plan

7. Set key dates for care plan and select Next
8. Edit the care plan as required
9. Select Next when you have personalised patient care plan

Approve Care Plan

If you are a nurse:

10. Approve the care plan on behalf of the patients GP select Approve on Behalf OR to release the care plan for approval by the GP select Release for Approval

If you are a GP:

11. Select Approve
12. Assign the individual allied health sessions and select
Next

The patient’s Inca record and care plan is now available to the care team members. You will be notified how the notifications are sent to team members, select Next


 Team Care Arrangement

13. Select the providers who have agreed to participate in the team care agreement and select Finish or;


14. If awaiting responses from providers, select
Finish. You may now exit the plan. If you do exit, your progress will be automatically saved
