GPMP/TCA Review: For existing Inca patients with a Patient-Centred care plan

This page will display the  GPMP/TCA review workflow for a patient with an existing Patient-Centred care plan in Inca

Both the GP and nurse workflows are identical
If the care plan is approved by the nurse, the GP must still log in to Inca to electronically sign the Individual allied health referral forms.
1.  Open patient record in Inca
2. Via the Overview tab, select  Commence GPMP Review
 If there is no option to commence review, please refer to FAQ article: How do I change the care plan review date?

3. Select care plan type then Next 


4. Confirm patient consent to create the review, then select Next 



5. Select the relevant consultation type, then select  Next.


6.  Tick off any tasks that has been Completed, enter any measurements that have not already been uploaded from your clinical software then select  Next 






 
6. Edit care plan contents as necessary. Actions marked as Completed in the previous step will be displayed with a green tick in front of that action in the care plan






7.  Add/Edit Medicare Rebated Visits  under the Team Members section as necessary

 



8. Edit or confirm suggested next review dates then select  Next 



9. A pop up will appear to confirm that the care team have been notified via their preferred Inca contact method



10. As per the Medicare requirements of a TCA/TCA Review, at least 2 care team members must agree to participate. Where you have obtained a verbal agreement from the care team member, you can agree on their behalf then select  Next 

 



The care team member must be assigned a task in the care plan in order to add their agreement

11.  Select  Next  to finalise the review
