GPMP/TCA Review: For patients with an existing Inca care plan

This page will display the GPMP/TCA review workflow for a patient with an existing care plan in Inca 

Both the GP and nurse workflows are identical

If the care plan is approved by the nurse, the GP must still log in to Inca to electronically sign the Individual allied health referral forms.

 

1. Open patient record in Inca

2. Via the Overview tab, select Commence GPMP Review

3. Select care plan type then Next

4. Confirm patient consent to create the review, then select Next

 5. Tick off any tasks that you have Completed, enter any measurements that have not already been uploaded from your clinical software then select Next

6. Select Expand All to view all goals and tasks and edit care plan content as necessary, then select Next

7. Distribute Medicare rebated visits to eligible allied health providers then click Next

Screen_Shot_2019-10-23_at_2.02.16_pm.png You may not receive this prompt if a Team Care Arrangement has already been created for the patient within the same calendar year. If the patient is eligible for additional visits and you do not receive a prompt, you can manually generate these via the Documents tab, once the Care Plan Review has been approved.

 

8. Confirm suggested next review dates then select Next

9. A pop up will appear to confirm that the care team have been notified via their preferred Inca contact method

10. As per the Medicare requirements of a TCA/TCA Review, at least 2 care team members must agree to participate. Where you have obtained a verbal agreement from the care team member, you can agree on their behalf

11. Select Next to finalise the review
