GPMP/TCA Review: For patients without an existing Inca care plan

This page will display the GPMP/TCA review workflow for a patient with no existing Inca care plan. 

Both the GP and Nurse workflows are identical.

If the care plan is approved by the nurse, the GP must still log in to Inca to electronically sign the Individual Allied Health Referral Forms.

 

1. Open the patient record in Inca 

2. Via the Overview tab, select Create Care Plan

3. Select care plan type then Next

4. Confirm patient consent to create the review and select Next

5. Select care plan conditions then Next

6. The care team will auto populate with your preferred providers. You can edit: mceclip0.png or delete: mceclip3.jpg these providers or select Add Care Team Member to search the entire Inca directory. When you are happy with your care team, select Next Screen_Shot_2019-10-24_at_9.14.32_am.png

mceclip5.jpgIf you cannot find a care team member in the Inca database, you can Register a New Provider. You will need to enter their first and last name, specialty, email address and phone number. You will also need to enter the details of their place of work

7. Select Expand All to view all goals and tasks. Edit care plan content to reflect the goals and tasks of the patient's previous care plan (completed outside of Inca) then click Next

8. Tick off any tasks that you have Completed, enter any measurements that have not already been uploaded from your clinical software then select Next

9. Select Expand All to view all goals and tasks. Update if necessary then click Next

10. Confirm suggested dates for the next review or edit as necessary, then select Next Screen_Shot_2019-10-24_at_9.27.02_am.png

 

11. A pop up will appear to confirm that the care team have been notified via their preferred Inca contact method

12. As per the Medicare requirements of a TCA/TCA Review, at least 2 care team members must agree to participate. Where you have obtained a verbal agreement from the care team member, you can agree on their behalf

13. Select Next to finalise the review

Screen_Shot_2019-10-23_at_2.02.16_pm.pngWhere a Team Care Arrangement has already been created for the patient within the same calendar year, the system may not prompt you to generate any individual allied health referral forms.

If the patient is eligible for additional visits, you can manually generate these via the Documents tab, once the Care Plan Review has been approved. See How do I update the Individual Allied Health Referral documents? for more information.
