How do I create a Referral Form for Group Allied Health Services?
The Referral Form for Group Allied Health Services can only be generated if:
- The patient has Type II Diabetes listed as an active condition in their Inca care plan
- The relevant Lifestyle task has been added to the care plan
- The health care provider responsible for the task is of the relevant specialty: Dietitian, Diabetes educator or Exercise Physiologist
Adding the task to the care plan
- Navigate to the Care Planning tab in the patient Inca record
- Ensure the care plan is set to Edit mode
- Click Add Goal
- Select the Lifestyle section
- Select the pre-defined goal Minimise diabetes lifestyle risk factors and click Add Goal
- Ensure the associated task Assessment for group allied health services has been assigned to the correct provider
Creating the referral forms
Note: If you added the above task to the GPMP before you approved it, the referral form will be created and the provider notified automatically at the point of approval
If the GPMP was already approved, you will need to follow the below steps to create and distribute the forms
- Navigate to the Documents tab
- Scroll down to Supporting Documents
- Click Create/Modify Supporting Documents
- Select the Referral form for group allied health services and click Create/Modify
- Click Create New and Create Document
- If a practice nurse completes the above steps, the GP will need to electronically sign the referral form.
