When a patient signs up for MediTracker, MediTracker will match the patient’s name, date of birth, Medicare number and mobile number are matched with the information stored in your clinical software. An exact match must be found before the patient can download their GP medical summary from your practice to their mobile device.
A patient’s GP medical summary is only created in MediTracker when they request it.
Precedence does not store patient data unless the patient consents to receiving the information and requests that it be retrieved from the GP clinical system.
Even if patients are disconnected from the Internet, MediTracker will continue to work. It securely stores the most recent copy of a patient’s GP medical record on their mobile device so it can be accessed online or offline, 24/7, anywhere in the world.
When you enable MediTracker at your practice, the patients who elect your practice as their primary care practice and purchase a MediTracker subscription will be able to access their GP medical summary.
If you wish to only offer MediTracker to selected patients, you can instead choose to use Precedence’s Inca service, which allows you to individually register patients for MediTracker. See FAQ below “What is the Inca Coordinated Care Platform?”
Patients can provide their consent to a family member or caregiver to access their GP medical summary by sharing their MediTracker username and password. For example, with consent, a family member could sign in to the accounts of their parents and their children.
