How to convert HCH care plans to GPMP+TCA’s?

1.      When the patient is due for a new care plan or a care plan review. Go to the Care Plan tab in the patient's Inca record. Then click on Rebuild Care Plan.

2. You will get the below options to select from.

- Select Convert current care plan to type:

- Then select GPMP + TCA (if it is a new care plan) OR GPMP + TCA Review (if it is a review)

- Once you have selected the action, click Next on the wizard and it will guide you through the rest of the workflow for the selected care plan type.
