Why is this patient on my patient list if I don’t have any sessions?

You can view and download your Allied Health referral form via Inca platform. Here is the instructions of how to view/download the EPC form.

 

On the referral form, it displays following information 

  • Number of session allocated to you
  • Contact details of the GP practice
  • Patient information

If you received an Inca notification of "Health Record is shared" but you don't have an EPC form available. It is possible that 

  • GP Management Care plan is not finalised and referral form is yet to be created. You can contact practice to find out more details 
  • The patient has a non-MBS care plan in Inca, in which you have been nominated as one of the their care team. You can use Inca to access their health record, view pathology results, enter process and record attended appointments.
