How do I change a care team member?

  1. Open the patient record in Inca
  2. Navigate to the Care Team tab
  3. Locate the care team member that you would like to replace
  4. Hover over the pen symbol in the Actions column to Change Provider. Any Preferred Providers listed in your account settings will be displayed in the short list. Alternatively, select Other Provider… to search the full Inca database

  1. Enter the name of the provider you would like to refer to and Search. Ensure that you have selected the correct specialty and state

  1. Select Assign to replace the previous provider with the new provider
  2. If the Allied Health Referral document needs to be updated once the care team member has been changed follow the steps for -
